Description

We are looking for a Care Coordinator to oversee the delivery of domiciliary care services. The ideal candidate will ensure that care plans are implemented effectively and that clients receive the highest quality of care.

Key Responibilities

Develop and implement care plans.
Coordinate care services with caregivers and clients.
Monitor and evaluate care services.

Qualifications

Diploma or degree in Social Work, Nursing, or a related field.
Previous experience in care coordination or a similar role.
Strong organizational and communication skills.
